Therapist Application Form

Simply fill out the form below and make your payment of £25. We will then request your references. Once both have been received we will invite you for a telephone interview. You can also download the therapist application form and return it by post with a cheque or postal order for £25.

In order to process your application we require therapists to pay a £25 registration fee, which will go towards the significant admin and staff costs involved in its processing, and also to allow us to continue our work with anxiety sufferers. Unfortunately, we will not be able to process your application unless we receive this. This fee applies to all therapists, including individuals looking to access a placement with Anxiety UK.

We expect that the application process should take no more than six months. The delays are usually at the beginning and end of the process whilst waiting for references and for CRB checks to come back. Please contact references in advance of your application to encourage them to feedback ASAP. We will try to contact you once per month as to the status of your application. Any applicants that have not contacted Anxiety UK within six months to inform us of the reason for any delay will have their application removed.

What happens after I apply?
  • Anxiety UK will request your references.
  • Once both references are received you will be invited for a telephone interview.
  • You will be asked to provide evidence of your qualifications, professional indemnity insurance, supervision arrangements and criminal record check.
  • Anxiety UK will contact you periodically to update you on the progress of your application (usually once per month).
